b. with an adult other than a parent/legal guardian, must have a letter of consent from a parent/legal guardian signed and sealed by a notary public. The letter must indicate the duration of stay, must identify the accompanying adult and must have the address and telephone number of this person. The letter should also identify an authorized adult who will receive the child upon arrival if different from the accompanying person, and must include the address and telephone number of this person.
NOTE: They must carry the original and a copy of the letter of consent/form.
